Great Heights requires a Licensed Clinical Social Worker who understands that quality, professionalism, and compassion are vital when working with clients and their caregivers. The Licensed Clinical Social Worker should have excellent communication skills, both oral and written, strong problem-solving skills, and efficient organization skills.
The position is Hourly, Monday — Friday 8 am — 4 pm full time and bi-lingual is a plus.
The Licensed Clinical Social Worker holds a minimum of a Masters in Social Work, and at least one year of experience working in human services.
The Licensed Clinical Social Worker will reflect and promote the values and missions of Great Heights when carrying out all workplace and community-based responsibilities. The primary role for the Licensed Clinical Social Worker are those duties necessary to assist families in the intake process and on-going coordination for the delivery of services by Great Heights.
- Communicates with prospective clients’ caregivers via phone and email
- Schedules and conducts tours with clients’ caregivers
- Assists caregivers in completing all intake paperwork
- Processes and files all client records from the intake process
- Assists families and facilitates case management services as needed
- Assists families and facilitates mental health therapy as needed
- Communicates needed information to the Billing Specialist during the intake process
- Attends all intake meetings
- Attends all initial Treatment Care Plan meetings
- Completes the Family Wellness Survey with caregivers upon intake, and provide the prescribed support based on the results
- Facilitates caregiver communication with Great Heights staff as needed
- Facilitates caregiver connection to community resources and supports as needed
- Provides direct caregiver training and group caregiver training.
- Facilitate staff connection to community resources and supports as needed
- Initiates regular communication and provides marketing information to referral sources
- Networks and collaborates with local mental health and human services organizations with the assistance of the Executive Assistant
- Regularly represents and promotes Great Heights at community promotional events that may be held outside of Great Heights’ regular business hours
- Communicates with the Executive Assistant regarding relevant newsletter information
- Oversees the Care Coordinator.
- Provides supervision for LSW’s hours and billing support.
- Maintains LCSW certification through continuing education and abiding by the ASWB’s ethical code of conduct
- Attends regular meetings with the Director of Clinical Services
- Attends all Great Heights meetings and trainings.
